Healthwatch Lincolnshire is dedicated to hearing about all experiences of health and wellbeing.
A large part of that is social care, so they are dedicating their next research project to hearing your experiences, good or bad.
Your input will help them to understand what is working well and what could be improved.

The survey should take around 5 minutes to complete and the information you share with them will help them to spot trends to identify areas for improvement.
They may use quotes in their reports, but they will not use any information that will identify you.
