Introduction to Social Prescribing
Lincolnshire Community and Voluntary Partnership employs Social Prescribing Link Workers on behalf of the Lincolnshire Integrated Care Board, as part of a wider programme of personalised care for Lincolnshire.
The Integrated Care Board is a statutory NHS organisation who are responsible for developing a plan to meet health needs of the population, managing the NHS budget, and arranging for the provision of health services in a geographical area.
Our role is to enable health and care professionals to connect people with community services and opportunities that support wellbeing.
What is Social Prescribing?
Social prescribing, sometimes known as community referral, is a way of enabling people to make positive changes in their lives and within their communities by linking people to activities, voluntary and community groups and public services.
Social Prescribing Link Workers offer one-to-one, person centred support and advice to individuals, helping them to connect with their community, reduce social isolation and improve their health and well-being.

What to Expect From Social Prescribing
Link workers can offer one-to-one support, to help people connect with what they are interested in.
Anything from arts/craft groups to activities that involve physical exercise, such as gardening and dance clubs.
It can also involve simply putting people in contact with services that can provide practical help and advice with issues such as debt, benefits and housing.

You can drop in to one of the social prescribing sessions for more information if you live in any of the sub-healthcare districts below.
TRENT PCN
These are held on the first Friday of the month at West Lindsey Leisure Centre (The Avenue, Gainsborough. DN21 1EP) in the foyer between 10:00am and 12 noon. There is plenty of free parking available.

Boultham Park - Moorland Park Methodist Church (46 Skellingthorpe Road, Lincoln. LN6 7RB) on the second Tuesday of the month between 10:00am and 12.30pm. Parking is available.
Birchwood - Birchwood BreakThrough Centre (Birchwood Library, Larchwood Crescent, Birchwood, Lincoln. LN6 0NA) on the second Wednesday of the month between 2:00pm and 4.30pm. Parking nearby is available.
Love Your Neighbour Café (The Salthouse, Free School Lane, Lincoln. LN2 1EY) on the second Thursday of the month between 10.30am and 1:00pm. Unfortunately there is no parking available at the location.
